About Our Assessment Process

SimplyHome’s assessment process is collaborative. Once an intake form is completed, we work with the individual, family, caregivers, and other providers to discuss and set the desired outcomes of using the technology as a natural support. We utilize a “best practices” approach to gather information and brainstorm solutions that capitalize on the individual’s strengths, abilities, and preferences.
A care provider introduced us to Omar, who was moving into his own apartment for the first time. As part of our process, the provider set up a conference call that included Omar, his care coordinator, Omar’s parents, the residential program manager, and a SimplyHome representative.
Each team member received an intake form prior to the conference call, so they could come prepared with preferences, priorities, and concerns. Everyone had the opportunity to offer feedback, including Omar. Afterwards the SimplyHome representative combined each person’s input into a system recommendation.
As a result, at the installation, no one was surprised by the technology being installed, how it would work, or how it alerted Omar, his family, and the caregivers. Because everyone had collaborated on the system design, very little adjustment was needed. Omar was able to move in right away and was fully supported by technology without additional caregiver support.
Focusing on the Person

That is, we focus first on the individual’s priorities, concerns, and interests, including the individual in the assessment process, and remembering that there are great opportunities for self-determination when it comes to residential living.
Integrating Technology with Provider Assessments
SimplyHome also provides Education and Consultation services to care providers who want to integrate assistive technology into their own assessment process. We help organizations understand how assistive technology can serve as a tool to promote better outcomes in the individual’s environment, routines and activities, abilities and health concerns, and community supports.
